Project description:
The Intensive Home Support service will run for 2 years and provide an alternative to hospital or short-term residential care admission for older people. It will provide a crisis response which is targeted at supporting people so that their on-going care become stable and predictable and can be met within their home setting, reducing the need for unplanned, inappropriate hospital admissions or longer-term residential care.

The service will offer intensive crisis support for a period of 24-48 hours, followed by a reducing level of care for up to six weeks, with a high degree of rehabilitative capacity, so that people are enabled to remain living in the community with an appropriate level of on-going support. The service will reduce the number of unplanned hospital admissions and residential care placements. Based on the target of 40 service users who would otherwise have received a residential placement, the potential annual savings are in excess of £600,000.

Similarly, there will be significant savings in costs related to hospital admissions, both in terms of the direct tariff costs and in the hidden costs resulting from disruption to existing care packages and the need to re-plan following admission and discharge.  The service will be able to support at least 40 service users who would otherwise have required a residential placement, and a further 150 who may otherwise have presented at hospital or been at risk of residential referral.
